Registering With the Surgery
When you move into the local area you can request to join the practice by visiting the surgery and requesting a new patient information pack. This contains a welcome letter from the Patient Services Manager, the Patient Information booklet, a new patient questionnaire and a GMS1 form.
Alternatively, you can complete the form online.
When you return these completed forms, reception staff will register you as new patients on our clinical system and a request to retrieve your clinical notes from your previous surgery will be made.
We ask that all patients attend the surgery for a 20 minute health check with the nurse to record relevant health information about you as it can take up to 8 weeks for the practice to receive your clinical notes.
If you are on regular medication your repeat items will be added to your clinical notes if we have the information but you may also need to see the doctor.
Please remember to update any other health organisations such as hospitals with your new address details and give them the name and address of the practice so that we receive all correspondence relating to you when you attend appointments elsewhere.

Temporary Registration
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
